• Case Management for Adults with a Traumatic Brain Injury (TBI)

Focused On
Your Recovery

A traumatic brain injury (TBI) impacts someone in a multitude of ways, both physically and mentally. Some people may develop a mental illness after the injury; for others, a pre-existing mental illness may become exacerbated. Working with a Daybreak case manager can help people with TBI’s to better manage daily needs while focusing on their recovery.

As they work with clients who have sustained a TABI and are dealing with mental health issues, they help to arrange services with the client’s chosen physicians, occupational health providers, mental health providers and rehabilitation therapists. When needed, they provide transportation to and from appointments and help with errands clients may not be ready to complete themselves. They also focus on helping each client regain as much independence as possible.
